Data Access Request

Patient or authorized person is entitled to make a data access request to obtain their copies of medical records or medical report through Medical Record Office. Application Forms can be downloaded via the link below. Please refer to the application form for more details and fees of relevant requests. Applicant should submit the written form to the Medical Report Office during the opening hour. For enquiries, please contact Medical Record Office.
